Who We Are
CRAN Integrated Medical Care is a house call medical practice registered in the state of Maryland. Over 56 million adults are ages 65 and older. Healthcare accessibility is a known deterrent in seeking healthcare, particularly for patients, who are frail and have limited mobility. Due to this limitation, most of these adults do not seek healthcare routinely for preventive services or routine follow-ups for the management of chronic diseases. This lack of follow-up results in emergency room visits that often leads to hospitalization. Our practice is focused on bridging this gap by bringing primary care services to patients in their own homes. The home medical model allows us to treat our patients’ symptoms and work with their caregivers/families to ensure ongoing support for the care plan. We collaborate/coordinate with other healthcare professionals to offer holistic quality care to our patients.
